In our dojang, when kids do their TKD belt tests they need their teachers to fill in a form for them, to see if they have been upholding the ideals of TKD in their daily lives. We've never had these forms, as our kids were home schooled. First time for everything!
First there's a scale that goes "excellent - very good - acceptable - needs to improve", then a space for comments.
