I have always read (or heard) "with all due respect" as a cynical comment, an intro where the hidden meaning was that the person speaking didn't really have a whole lot of respect for the listener but that the listener was a person of position.
Sometimes. But I know personally that sometimes I use the phrase in an attempt to politely express a difference of opinion. A version of YMMV as it were. In my opinion, not all differences of opinion are meant to be taken as disrespectful.
